Focus on Prevention

The P in SIMPCO is for prevention, and it’s never silent. SIMPCO gives you the ounce of prevention that’s worth a pound of cure. Our goal is to keep you healthy, so you don’t have to try to get well. Join us and let us show you how to increase the quality and quantity of your life by becoming better informed on how to become a better steward of your own health!

Individually Tailored Health Plans

With SIMPCO, everything is about you, and nothing is done without you! We will construct preventive health plans designed around your unique opportunities and challenges. Our team will be accessible and available to answer your questions, provide you advice, and direct you to what’s needed for your wellbeing. The SIMPCO Health and Human Service Centers (Health Hubs) will be available 12 hours a day, providing you education and other services and you will receive quarterly health visits with our personal health consultants. Additional needs will be accommodated by the SIMPCO Call Center and Telehealth Services.

Navigators (Administrative Personal Health Consultants), Digital Support

SIMPCO will coordinate your health care and direct your preventive care strategies. Our Administrative Personal Health Consultants (aPHCs) will keep you enrolled, credentialed and connected with your medical home and community-based resources you need to be the healthiest version of yourself! The SIMPCO Call Center and Telehealth Services are available around the clock to address your questions and health inquiries (including mental health questions).

Clinical Personal Healthcare Consultants

SIMPCO’s Clinical Personal Health Consultants (cPHCs) are your health coaches! You won’t have to wait for an annual visit. Your cPHC will meet with you quarterly to design your plan, check your progress, and provide you with solutions to keep you healthy and productive!

Participant Empowerment

Everything within SIMPCO is about empowering you. We will provide you with the tools and support needed to navigate your health journey. We will be the resource and trusted messengers you need to have accurate information and experts available. Once you make the commitment to prioritize your health, SIMPCO is your partner on your health journey.

Health and Human Service Centers

The SIMPCO Health and Human Service Centers (aka Health Hubs) are preventive health campuses located within your communities and virtually via your computer, tablet or phone. Ongoing Communication and Coaching

SIMPCO is your active partner in your health journey. Your Administrative and Clinical Personal Health Consultants are available to you when you need them and before any crisis arises. You can visit a SIMPCO Health Hub every day should you choose! The SIMPCO Call Center and Telehealth Services are available around the clock to plan your visits and strategies, and to coordinate services with your medical home.

TeleHealth

SIMPCO Telehealth is available around the clock to provide you information and advice, whether you need it urgently or routinely. SIMPCO Telehealth also provides mental telehealth services for our clients. We’re just a phone call or key click away!

Mobile Units

For SIMPCO clients who have trouble accessing our Health Hubs, we can accommodate your needs via virtual visits or via a SIMPCO Mobile Unit. Ask your Personal Health Consultant for details and qualifications for this additional service.
